Details: SUMMARY: Violation investigation - Non reporting Bac-t 4th quarter 2007 DETAILS: Phone call to system operator. Spoke with Rachel Myrick. Test had been done in October 2007. Results were sent to Curry Co Health Department. No copies sent to DHS. Operator assumed that county would forward. Violation number 35181 ACTION NEEDED: County will forward results to DHS. DHS, please credit system with results.
