Details: SUMMARY: Monitoring violation DETAILS: 11/4/08 I contacted Vonnie about the overdue monitoring. It should have been done by 12/31/08. I informed her that IOC monitoring qualifies to be reduced to once every nine years. I have submitted the chemical change form. Vonnie said the overdue chemical monitoring is scheduled go be done this November 17th ACTION NEEDED: Follow up on monitoring
