Details: SUMMARY: TCR M/R violations for 3rd quarter 2008 DETAILS: 10/22/08-- left message for operator. Mailed letter outlining violation and required corrections. 10/22/08: Operator returned phone call and said they were confused about the quarterly schedule. We discussed the dates for the quarterly monitoring schedule. A sample for the 3rd quarter was not taken. ACTION NEEDED: Take quarterly samples and submit on time.
