Details: SUMMARY: Failure to submit third quarter coliform DETAILS: 11/3/09 Called and left message for Travis. 11/5/09 I called again and asked Sue to inform Travis I called and to bring a fourth quarter sample into the lab ASAP. Travis called and informed me that a fourth quarter sample has been submitted. 11/16/09 The 11/5/09 sample tested negative per Neilson Research. ACTION NEEDED: None
