Details:	SUMMARY: Alert COLI6561 - 4 of the 4 repeat samples collected on 8/4/09 tested positive for total coliform. DETAILS: Jay Spolski called on behalf of Mr. Smith. Jay informed me that 4 repeat samples tested positive for total coliform. Orchard Glade took corrective action last night: a gallon of bleach was dumped and recirculated in the well-- two brand new storage tanks and the existing storage tank were chlorinated too. System users have not yet been notified that the system is chlorinated and therefore adequate chlorine contact time to disinfect the distribution system is a question mark at this point. Jay agreed to inform system users that the well and tanks have been chlorinated and to try to attain 8 hours of contact time in the distribution by having system users open taps until they smell chlorine then leave the taps closed for the 8 hours. I emailed Jay a tier two public notice template and requested that he return the template after it is filled out. 8/7/09 - I spoke with Mr. Smith today. He says the chlorine level in the distribution system is zero and will be taking a special sample this week. 8/19/09 -Jeff called. He said a special sample was collected and came up positive for total coliform. We discussed shock chlorination procedures and he says they will shock the entire system asap, probably tomorrow. 9/22/09 - One special sample submitted tested negative for total coliform. ACTION NEEDED: Five temporary routines need to be collected in September.
