| Details: | SUMMARY: Source assessment (SA) DETAILS: Call from Alexin Analytical Lab asking about the reporting route for September coliform result that happens to be positive, given the operator was told to send all his monthly lab samples to the state office in Springfield. Lab reporting form does not have the source sample type checked off. Also, August reporting form marked special this not submitted to the state by Alexin. Called Brad Daniels to clarify where the SA reports for this system are to go. Informed it is ok to send all to the Portland office for data entry. Called Steve to clarify where the August and past lab samples marked either Assessment or special were taken. August was at the well head, in addition to a December 22, 2010 was at the well head. Both reporting forms were marked incorrectly. Steve to call Alexin to verbally correct and ask for those two reporting forms to be changed to indicate Source Assessment. Alexin will submit in their normal reporting process all SA results to the Portland office. System has done 6 SA samples to date. ACTION NEEDED: Monitor for monthly SA sampling given notice of non compliance from state dated 12/1/2010 | ||
