| Details: | SUMMARY: Lead and copper (LCR) not valid DETAILS: spoke to Paul 8/22/11 regarding the LCR excursion alert received on 8/18/11. Paul sent the logs for Ph and Phosphate via email. No excursions were noted. Upon further review by Chuck Michael it was discovered that Paul had written the wrong system number on the form. He wrote 4191712 and the correct number is 4193712. It was a coincidence that the facility number he listed was a school in Lebanon that had corrosion control in place as well. The issue was corrected. It should be noted however that Paul is taking distribution samples when he should be taking them at the entry point. I told him distribution samples should be submitted when conducting lead and copper sampling. ACTION NEEDED: Insure system is taking and submitting the proper entry points samples for Ph and Phosphate on the correct log each month. Insure distribution samples are submitted when taking lead and copper samples and they are submitted on the correct log form. | ||
