Details: SUMMARY: Significant deficiency follow up DETAILS: Two deficiencies from 5/3/11 survey corrected and pictures submitted. No screen on vent. A screen has been installed on the well casing vent tube. Opening on the wellhead at the electrical conduit. This opening has been sealed with a silicone material. ACTION NEEDED: none
