Details: SUMMARY: Violation notice DETAILS: System received violation notice regarding lack of repeat samples and public notice occurring in July/August 2011. Firm submitted 5 routine samples in August 2011 and completed routine 4th quarter sampling for return to compliance. I discussed the notice and system violations with Casey Lyon of the state. I informed Mr. Shires no further action was required. ACTION NEEDED: none
