| Details: | SUMMARY: Monthly disinfection report monitoring update DETAILS: Spoke with Lori and she said Delta Environmental Labs mailed the monthly report at the end of December 2011 and she received her copy. I gave Lore the PO Box 14450 address in Portland to make sure the form went to the correct address and also the fax number to the Portland office. She said she would send a new copy today. ACTION NEEDED: Submit another copy to Portland office. | ||
