Details: SUMMARY: Follow up to deficiencies an sanitary survey DETAILS: Contacted operator and deficiencies are not corrected. I placed the system on a corrective action plan. We agreed they would have the items corrected by March 1, 2012. I sent the guidance documents again. See attached letter. ACTION NEEDED: none
