Details: SUMMARY: Sanitary survey deficiency follow up field visit DETAILS: Field visit to verify corrections from 5/3/11 survey. No screen on vent and wire port on well head not sealed. Met with operator Doug and looked at improved well head which now has a screened vent and the electrical wire port has been sealed. ACTION NEEDED: none
