| Details: | SUMMARY: Site visit DETAILS: The drain pipe from both reservoirs were observed to not have screened covers on 5/21/12. This deficiency was observed corrected today. 5/21/12 it was observed that an undocumented/unapproved well was observed within the 100 hazard zone near source well #2. Today it was observed that the well seems to be constructed with an adequate sanitary seal. It appears to be a pit less adapter style well. The operator presented a monitoring form for ensuring that chlorine residual is tested in the distribution system at least twice weekly. They have reportedly begun monitoring chlorine residual as required by rule. In a letter date 6/14/12 the operator has demonstrated that written protocols are in place for under certified operators. ACTION NEEDED: The roof has loose bolt/screws causing a potential entry for rain water that may be contaminated with animal feces. The styrofoam plugs that seal the openings of the steel roof material against entry of small animals was not properly packed into the holes. In some cases it was hanging from the roof openings leaving large openings that could easily be accessed by a small rodent or bird. Cross Connection Control - Testing and report submission.
