Details: SUMMARY: Failed to report VOC quarterly DETAILS: I contacted Melvin and informed him of the violation He said he was unaware he had to do 4 quarters of VOC testing. I also discussed lead and copper violation. I reminded him to do a public notice for the violations and send in copy to the state. ACTION NEEDED: Do required testing for 4 quarters total for VOC. Public notice required.
