Details: SUMMARY: Correction of significant deficiencies noted on 11/7/12 system survey DETAILS: Thirty days have passed since the system survey report was sent to the operator. Our office had not been consulted about correcting the significant deficiencies noted on the survey. I sent a letter to the operator reminding her of the 18 week correction time frame. ACTION NEEDED: Significant deficiencies need to be corrected by 3/20/13
