Details: SUMMARY: Significant deficiency (SNC) DETAILS: 6/1/12 survey showed no ASR, no EA, no CCR, no O and M manual and no ERP. System worked with me and HBH (Doug Wise) to develop all of these mission documents. Package of all missing docs was submitted 1/28/13. All appear to meet criteria. ASR is being prepared and will be submitted to the state by March 31 ACTION NEEDED: Submit ASR this year and each year by 3/31
