Details: SUMMARY: Survey deficiency follow up DETAILS: Deficiencies noted during 8/20/10 survey included no coliform sampling plan, no enabling authority and no annual summary submitted. Worked with Butch and Nate for a couple years and Nate in office today to submit missing documents. Submitted signed EA, annual summary for 2012 and coliform sampling plan. ACTION NEEDED: Will schedule survey for 2013 ASAP. Continued on time reporting and maintain all required documents and DRC/contract.
