Details: SUMMARY: Resolution of violations DETAILS: System failed to take monthly source assessment samples as SRC-AB numerous times in 2012. I had emailed Magar on 4/13/12 to have him sample monthly at SRC-AB Magar has been ill and it was still not done. Curt was hired back at the MHP and the source sampling was done 11/9/12 ACTION NEEDED: Monitor and be sure system maintains monthly assessment sampling
