| Details: | SUMMARY: Respond to repeat coliform DETAILS: Donald called the office on 4/15/13 to discuss the alert and at that time informed me the repeat samples came back with 3 positive for coliform and the source was negative. Donald decided to turn on the chlorinator on Friday and had residual chlorine in the system over the weekend. Because the repeats were positive a tier 2 notice is required which I emailed him the form and once filled out to email or fax back to me. I also let him know that next month 5 more samples will need to be submitted. Because Donald had a few questions I did no know the answer I gave him Casey Lyons cell number to discuss the best options for him to follow. ACTION NEEDED: Tier 2 notice filled out and sent back with in the next 30 days as well as 5 more samples will need to be submitted in May 2013 | ||
