Details: SUMMARY: Source assessment sample DETAILS: I contacted Darrell by phone who represents the golf course. I explained the violation to him. He said he was not aware he needed to take a sample from the source and never has. He will take a sample from the well head right away and do a public notice and send in copy to the state. ACTION NEEDED: Source assessment sample needed right away. Public notice an send copy to the state.
