Details: SUMMARY: Survey Followup Documentation DETAILS: Following up on deficiencies from the 2011 survey at Ikes Pizza. The system at the time did respond with a letter from there lawyer and follow up e-mails stating that all deficiencies were corrected. I have copied these documents and sent them to the state. Following up with a contact report for doumentation on SDWIS. ACTION NEEDED: None
