Details: SUMMARY: Sanitary survey follow up DETAILS: Deficiencies noted on survey: Chlorine residual in distribution system not recorded twice a week. Annual Summary report not submitted to the state. No ordinance of enabling authority for cross connections. Annual CCR not submitted to the state. ACTION NEEDED: Placed on corrective action plan for deficiency #3 on 4/1/14. All other deficiencies were corrected.
