Details: | SUMMARY: missed GWR 18-week deadline for correction of significant deficiencies DETAILS: Left messages last week and again today-- Erica Jo phoned me back today. The 18-week deadline for corrective action (stated in the February 6th survey report) was June 12th, so shes missed it. Although we had discussed progress on the significant deficiencies in March, she was going to follow up with a letter stating what had and had not been done. I never received any written confirmation of corrections, and as I understand it, the Operations and Maintenance Manual is still unfinished. I explained Erica that now a Tier 2 public notice is required, and I would send her the correct template. I emailed the correct template to her later today, along with the wording from the February 6 letter that explains how to send us a copy, and the names the significant deficiencies that were to be addressed. ACTION NEEDED: Await copy of the Tier 2 notice, and most likely a request from Christmas Valley to approve additional time (a corrective action plan) to address the final significant deficiencies. |
