Details: SUMMARY: Investigation for violation # 902425413 & 902425412 DETAILS: I contacted the operators wife & let her know to pass along to sample at the well head. Since the system chlorinates they need to take an annual source sample. She said that she understood & would pass along the message. ACTION NEEDED: Monitor & ReturnTo Compliance
