DETAILS: Operator failed to remove aeration device from tap prior to flushing and taking sample. ACTION NEEDED: Advised proper sampling technique. Take 4 repeats on September 30th, and 5 temporary routines during the period of 10/1 2014 - 10/31/ 2014.
