Details: SUMMARY: Significant deficiencies noted on the 6-30-14 survey have been corrected. DETAILS: 12-18-14 --- Significant deficiencies noted on the 6-30-14 survey have been corrected: Ty came in the office today and informed me a sample tap has been installed at the well head (photo was e-mailed this afternoon) and brought in a copy of the O&M Manual. The ERP was e-mailed this afternoon. ACTION NEEDED: Action has been completed.
