Details: | SUMMARY: Comprehensive Performance Evaluation being conducted by DWS for NTU exceedences DETAILS: Due to Midlands treatment plant exceeding filter effluent turbidity triggers from September through December 2014, DWS is conducting a Comprehensive Performance Evaluation (CPE) at the plant. Midland requested DWS conduct the CPE rather than a 3rd party. I, together with Evan Hofeld and Fred Kalish of DWS, met with Scott MacGregor, board president, and Phil White, treatment plant operator in direct responsible charge on 1/14/15 for the first day of the CPE. We discussed the reasons for the CPE, and began gathering data on the treatment plant, information on plant operations and maintenance, as well as adminstrative information. The DWS team will be conducting the CPE over 3 site visits in mid to late January, 2015. DWS will prepare a CPE report for Midland by the end of February 2015, or by the required 120 days after the first turbidity trigger outlined in OAR 333-061-0040(1)(d)©(III) in late October 2014. ACTION NEEDED: DWS to compile information and prepare the CPE for Midland by the end of February 2015. DWS will present summary of report, including performance limiting factors to Midland in an exit interview to be scheduled upon completion. Midland and DWS will discuss Midlands plan and schedule to resolve the performance limiting factors outlined in the report. |
