Details: SUMMARY: Dana Petersen DETAILS: I left a detailed message on the water operators voice mailbox concerning the letter I sent out 2 days ago concerning the Significant deficiencies found during the last water survey on 08/20/2014. I told him that I would e-mail him the templates again and that I needed him to send back letters of confirmation in regards to the emergency response plan and operations and maintenance manual. ACTION NEEDED: Correct deficiencies
