Details: SUMMARY: Final significant deficiencies addressed for 2014 survey DETAILS: Received an email on 5/4/15 from Suzanne, the regional LDS Admin Assistant in Bend, that the Coliform Sampling Plan, ERP and Operations and Maintenance Manual for the Lakeview Chapel had been finalized and approved by the LDS headquarters. The one-time arsenic result was received on 3/30/15. The 2014 nitrate result was re-submitted from the lab on 5/4/15 and entered into the database. Meanwhile, their plan review approval was finalized on 3/17/15 for the change in storage tank arrangements (which occurred after the survey date). ACTION NEEDED: None at present
