Details: SUMMARY: Significant Deficiencies Follow-up DETAILS: 1. Well setbacks from hazards. Oil tank within 12 feet of the well.* Tank removed but soil still needs to be excavated. There is a second tank, but it is within 300 feet of the well-adequate distance from well.2. No Operation and maintenance manual.*O/M still incomplete, requires a schematic and list of daily, weekly, semi-annual, quarterly and annual maintenance of the well. 3. No Emergency Response plan.*DEFICIENCY RESOLVED ACTION NEEDED: Locate the well log (if available) for estimating disinfection calculations.DEQ certificate of completion of project and a completed O/M is required by revised due date.
