| Details: | SUMMARY: Total Coliform Alert DETAILS: Called Kevin OLeary regarding all repeat samples collected 5/3/17 showed the presence of coliform bacteria. He stated that he found the line providing the chlorine feed into the tank was not intact and that he had fixed this over the weekend, although chlorine residual seemed ample in the samples collected on 5/3 he speculated if the bad line provided an opening for contamination into the system. I explained that he would receive a Level 1 Coliform Investigation Form in the mail within a few days and encouraged him to complete it promptly and send it to me. Expect this to be done within 10 days. ACTION NEEDED: Complete level 1 investigation form and get completed form to CHW. | ||
