Details: SUMMARY: Level 1 Investigation Violation DETAILS: John called to find out why he received a postcard in the mail about a violation. I reminded him the last time we talked I explained that he would need to do a Level 1 and sent him the form via email. He was going to stop by the office today to pick the form up.
