Details: SUMMARY: Deficiency Corrections DETAILS: Contacted operator to let them know their deficiency corrections deadline was approaching. Andrew requested an extension to complete these activities and we made an appointment on the 29th for an office visit so that I can help him with completing the necessary requirements. Requesting an extension to complete the deficiencies within 90 days. All deficiencies will be corrected by December 14, 2017. ACTION NEEDED: Meet with operator on 09/29/2017.
