Deficiencies noted are as follows: 1. Wells share raw sample tap. Wells do not meet required 100' setback from hazards. 2. Free chlorine residuals are not recorded daily as required. 3. System has no operation and maintenance manual. System has no written operating protocols for other operators. 4. System is monitoring for coliform bacteria at treated sample tap and not within distribution system. 5. Required monitoring not current. Confirmation sample of di(2-ethylhexyl)phthalate not monitored and reported.
