  Comments:
Source - Well head seal not watertight. M&R - No Coliform Sampling Plan. Management - No Emergency Response Plan, No Operations and Maintenance Manual. RECOMMENDATIONS: Raise wellhead to 12" above ground, fix crack in concrete slab, put screen on outlet of pump-to-waste pipe, replace lock on pumphouse.
