Monitoring Deficiencies: lead and copper tap sampling overdue; no written coliform sampling plan. 3. Management & Operations Deficiencies: no operation and maintenance manual; no emergency response plan. 4. Operator Certification Deficiencies: no operator of record; no DRC. 5. Other Rule Violations: CCRs are not being issued.
