  Comments:
The significant deficiencies and rule violations noted are as follows (see http://www.oregon.gov/DHS/ph/dwp/docs/pwsrules/pwsrules.pdf for a copy of the drinking water rules): 1. No quarterly routine coliform samples (see OAR 333-061-0025(1)). 2. No yearly nitrate sample (see OAR 333-061-0025(1)). 3. No initial arsenic sample (see OAR 333-061-0025(1)). 4. No coliform sampling plan (see OAR 333-061-0036(6)(b)(F)). 5. No operations and maintenance manual (see OAR 333-061--0064(1)(a)). 6. Emergency response plan not completed (see OAR 333-061--0064(1)(a)). In addition, I have the following comments and recommendations: 1. Provide a current well log, upgrade or construct a new well according to Oregon Water Resources Department, Well Construction Standards (see Oregon Adminstrative Rules, Chapter 690, Division 210 or http://arcweb.sos.state.or.us/rules/OARS_600/OAR_690/690_210.html). 2. Recommend that the Lakeridge Country Club check with the Oregon Water Resources Department (phone: 503-986-0900) regarding water right requirements.
