| Comments: The significant deficiencies and rule violations noted are as follows: Management and Operations: 1. No Operations and Maintenance (O&M) Manual. This is a requirement for all public water systems and should include routine tasks that you perform at the wells, storage facilities, and in the distribution system. This should be made available for review during subsequent site visits and surveys. Please see attached outline of key required elements of an O&M Manual. Operator Certification: 2. Develop a written Operator Protocol for all operators not designated as the operator in Direct Responsible Charge (DRC). This protocol should list what procedures operators can do and under what circumstances the DRC should be consulted. This procedure should be signed by all operators and submitted to the DWP. Please refer to the template protocol provided during the survey. Distribution System: 3. No certified cross-connection control specialist. Our records at DWP indicate that your certification as a specialist expired on June 30, 2005. Please take the training course and re-apply for certification.
