  Comments:
No coliform sampling plan. No operations and maintenance manual. Recommend: Replace small mesh screening at reservoir that has holes. Add small mesh screening to bottom portion of air gap at reservoir wall where roof meets concrete. Locate all dead ends and conduct routine flushing two times per year. Create a service area and facility map. Clean reservooir. Recommend install pipe off valve to create drain to daylight. Update emergency response plan.
