Well casing is not 12" above concrete slab. No raw water sample tap. Well is whithin 18' of septec line from tank to drainfield. The chlorination system was not approved by DWP No coliform sampling plan No nitrate sample since 2006 No public notice for nitrate violation No operation and maintenance manual
