Storage tank lind not locked / secured. Coliform samples due in May 2009 not collected. Nitrate sampling not up to date. Violation on 2-16-2010 not resolved. Coliform sampling plan not completed. Emergency response plan could not abe located. Rule violation - No concrete slab around well casing - recommend pour concrete slab.
