  Comments:
The significant deficiencies and rule violations noted are as follows: 1. No operations and maintenance manual (see OAR 333-061-0065(4)). In addition, the following comments and recommendations are as follows: 1. Recommend that the water system update their coliform sampling plan using the newly revised Coliform Sampling Plan template (see Attachment 3).
