  Comments:
The significant deficiencies and rule violations noted are as follows: 1. No raw water sample tap (see OAR 333-061-007). 2. A flap valve and/or screen needs to be installed on the reservoir overflow (see OAR 333-061-0076). 3. Water system needs to complete an Operations and Maintenance Manual (see OAR 333-061-0065(4)). In addition, I have the following comments and recommendations: 1. Recommend that the water system collect a new arsenic sample since the MCL has changed from 0.05 mg/L to 0.01 mg/L. The reason for recommending that a new sample be collected is that Minimum Detection Limits (MDLs) for the samples collected in 1991 and 1994 are unknown and may be above the new MCL. 2. Recommend inspecting the condition of the reservoir interior and clean if needed. 3. Recommend placing the reservoir on an inspection and cleaning schedule. 4. The recently adopted Ground Water Rule required that the Drinking Water Program revise the Coliform Sampling Plan. Recommend that the water system revise their existing plan using the new template (see http://public.health.oregon.gov/HealthyEnvironments/DrinkingWater/Documents/gwater/csp_template.doc.
