The deficiencies noted are as follows: 1. It is unknown if the well meets setbacks from sewer lines. 2. Chlorine residuals are not measured and recorded daily. Chlorine bleach is not NSF approved. 3. Screen on drain/overflow broken. 4. No written coliform sampling plan. 5. No O&M Manual. No ERP. No Plan Review for Chlorination system. 6. No written standard operating protocols for other operators.
