Details: SUMMARY: Significant deficiencies DETAILS: The significant deficiencies noted on the 11/8/11 survey report have been corrected. The 2010 Annual summary Report is now noted as received in the state website. I received a copy of the written coliform sampling plan, Emergency Response Plan and a 2010 CCR certification form. I forwarded the 201 CCR form to the state. ACTION NEEDED: none
