Details:	SUMMARY: Request for Technical Assistance - L&C M&R & Other Community System Requirements DETAILS: We met with Mr. Zastrow in our office and together we reviewed the proposed sample sites for L&C M&R. Then we completed Form 141A for each of the proposed sample sites. ACTION NEEDED: By June 30th, 2016: Complete the first round of lead & copper sampling.Between August 1st and August 31st, 2016: Compete the first round of Disinfection By-Product monitoring.By March 31st, 2017: Submit the first cross connection control ‘Annual Summary Report’ to the Oregon Health Authority (along with the appropriate fee).By July 1st, 2017: Publish the first Consumer Confidence Report (CCR) and deliver it to your customers.By 12/31/2018: Collect and analyze a sample for Asbestos within the next three years.
