| Comments: The recommendations and comments noted are as follows : 1. Install a raw water sample tap in front of the phosphate injection site. 2. Record water phosphate concentration at least 2X weekly. 3. Service the screened vent on Well #3 casing so it is facing down. 4. Update the coliform sampling plan so it meets the current GWR requirements.
5. Locate the source of the Nitrate contamination affecting well #1.
